Social Hx:

Smoking History - Current Smoker, 1 packs per day.

Alcohol Use - Heavy alcohol intake, drinks every day, ~17 drinks per day.

Illicit Drugs - Current IVDA, cocaine, heroin. Denies needle sharing.

Marital Status - Married

Physical Exam:

General: Patient comfortably seated, slightly diaphoretic.

HEENT: Tympanic membranes congested, clear discharge.

Chest: 1+ rales, dull to percussion.

Abdomen: LLQ point tenderness, diminished bowel sounds.

Extremities: Poor capillary refill, RLE 1+ DP, faint PT.
